* YOU MUST HAVE THE FOLLOWING DOCUMENTATION TO SIGNUP:
¨ Proof of Income You must provide documentation of ALL household income (pay
check stubs, Families First, SSI or Social Security documentation, or Food Stamp
documentation: must provide Food Stamp letter.
¨ Social security card for head(s) of household
¨ Social Security Card or Birth Certificate for any child 12 and under you wish to receive toys
Proof of Residency (electric bill, water bill, gas bill or home phone bill)
*If applying for toys, anyone other than parent must show proof of guardianship.
