YOU MUST HAVE THE FOLLOWING DOCUMENTATION TO SIGNUP:
* Proof of Income: You must provide documentation of ALL household income (paycheck stubs, Families First, SSI/Social Security documentation, & Must Provide Food Stamp Letter.
* Social security card/ Birth Certificate for head(s) of household and any child 12 & under,
if you wish to receive toys.
* Proof of Residency: electric bill, water bill, gas bill or home phone bill
*If applying for toys, anyone other than parent Must show proof of guardianship.
